Researchers at the MacAnxiety Research Centre are conducting this survey to investigate sound sensitivities and how they relate to a number of demographic and psychological measures. This page describes the purpose and methodology of this survey. The study is being conducted by Dr. Michael Van Ameringen, and the study is not funded. PURPOSE OF THE STUDY
You are being asked to complete an online self-report survey. The survey is designed to investigate sound sensitivities to determine how common these sensitivities are and how they relate to other psychological measures.
WHAT DO I NEED TO DO?
After reviewing the information provided in this document, and checking off the consent box at the bottom of the page, you will be able to complete the online self-report survey. The survey will take approximately 60 minutes to complete. As part of the survey, you will be asked to provide basic demographic information (e.g. age, gender, and race) so that we are able to evaluate the information for relationships between these demographic measures and other data collected. All survey data will be completely confidential. Upon completion of this survey, you may be invited to participate in additional research conducted by members of this study. Participation in this survey, and in additional research, is completely voluntary. 
WHO IS GOING TO SEE MY ANSWERS?
Your responses will be completely anonymous. Your name will not be linked to your responses in the electronic database; your responses will be linked to your study number only.
WHAT ARE MY RIGHTS AS A PARTICIPANT?
Participating in this study is entirely voluntary. You may choose not to participate, or to withdraw participation at any time during the survey. This study has been reviewed by the Hamilton Integrated Research Ethics Board (HIREB). The HIREB is responsible for ensuring that participants are informed of the risks associated with the research, and that participants are free to decide if participation is right for them. If you have any questions about your rights as a research participant, please call the Office of the Chair, Hamilton Integrated Research Ethics Boards at 905-521-2100 x 42013. The research project and the procedures associated with it have been explained to me in the description above. The experimental procedures have been identified and no guarantee had been given about the possible results. My participation is voluntary and I do not have to consent to this form if I do not want to be part of this research study.
